PLEASE KEEP THIS SHEET FOR YOUR INFORMATION
IMPORTANT INFORMATION FOR PATIENTS SEEKING TREATMENT


Before any initial assessment appointment can be made we must have received the following:

    1. PERSONAL HISTORY FORMS
    Both the enclosed Personal History and Information forms completed in full.

    2. (FOR OPIATE MAINTENANCE TREATMENT PATIENTS ONLY - NOT SUBUTEX DETOX OR IMPLANT PATIENTS) DOCUMENTED PROOF OF ABILTY TO PAY FOR YOUR TREATMENT.
    The Home Office requires this. Proof can be two recent wage slips or bank/building society statements showing regular income. STATE BENEFITS CANNOT BE ACCEPTED AS FUNDING FOR YOUR TREATMENT. If you are not working, you will need a guarantor, in which case they will need to provide us a signed letter stating their willingness to pay for your treatment. They will need to provide the same proof as above and an advance payment of one month’s treatment fee. This payment will be held by us and only used in the event of you being unable to pay for a prescription on time. This should only be seen as an emergency fund to prevent you becoming in debt to us. Therefore, if any of this fund is used, you will be required to repay the amount used in order to remain a month in hand.

    3. A DEPOSIT PAYMENT
    The full cost of assessment for methadone or other opiate maintenance and alcohol treatment is £250, for which a £150 advance payment is required. Cheques /Postal Orders to be made payable to: The Stapleford Centre. We also accept VISA, MasterCard, Switch, Delta, Electron & Solo.
    The full cost of an assessment for Subutex (Buprenorphine) detox is £250 for which a £150 deposit is also required.
    All appointments for naltrexone implants also require a deposit payment of £150.

    4. GP and CHEMIST DETAILS
    The name, address, telephone number of your G.P. and a Chemist convenient for you who is willing to dispense for you and stocks the medication you require. There is a space for this on the enclosed Information form.

    5. Four colour passport photographs of yourself
    Please post these items to us and allow sufficient time for them to arrive. Then telephone us to arrange an appointment. To protect your confidentiality, we do not telephone new patients to arrange appointments.





INITIAL ASSESSMENT

A methadone or other opiate maintenance assessment could take nearly all day and therefore will be booked for early morning (9.30 or 10).

Subutex or Rapid Opiate Detoxification assessments can take up to 2 hours.

Please remember to cancel your appointment with as much notice as possible if you are unable to attend. If you arrive late we will not be able to see you and you will have to re-book. Our schedules are very busy so the next available appointment is unlikely to be in the same week.

Assessment for methadone or other opiate maintenance will require you take a test dose of medication in the presence of a doctor, so please attend as drug free as possible. Please remember to bring enough money to cover the chemist charges (usually no more than £20.00) as well the remainder of the consultation fee. Maintenance patients will be required to return every 3-4 days for the first few weeks as their initial dose (of no more than 50mg of methadone or equivalent for morphine patients) is titrated upwards over a few weeks. Each of these visits will require a further test-dose until you are stable.

Following assessment for subutex detoxification, you will be required to return to the clinic 3-4 days later to assess your progress, and thereafter, at least every week until you are detoxed.

As part of all assessments you will be required to provide a urine sample and random testing will aslo be carried out during treatment. (If you are prescribed certain categories of drugs you will have to pay for the tests regardless of the results)

Receiving private prescriptions means that you will have to pay the full cost of your medication at the chemist instead of the NHS dispensing fee. The costs vary depending on the type and preparation of medication. It is worth checking the price at different chemists as price can vary significantly from chemist to chemist. Please remember that chemist costs are totally separate from our charges and you (or your guarantor) are solely responsible for both.

Fees
Maintenance fees depend on the type of medication (i.e. oral or injectable) you are prescribed, and the length of prescription, which will be decided by your doctor.

If you do not attend your regular appointments for more than one month without notice or good reason, we will assume that you are no longer in treatment with us. Therefore, if you contact us for further treatment you will be treated as a new patient and will have to re-register accordingly.

NB The clinic is also open on Wednesday evening. This session is exclusively for those patients in full-time employment who can demonstrate to the clinic's satisfaction that it would be extremely difficult to attend during the normal hours.

HOW TO GET AN APPOINTMENT
When you have filled in the Personal History Forms, put it in the post to us along with £150 advance payment for your initial appointment (payable by cheque to ‘The Stapleford Centre’ or by credit card over the telephone). A day or two later, telephone us to make sure we have received the form and payment and then we can make an appointment for you.

Please make every effort to be on time by leaving plenty of time to travel (especially through the centre of London) as you will have to rebook if you are late and bear in mind that the next available appointment might be a further 2-3 weeks away.

RE: NALTREXONE IMPLANTS
If you are planning to have a further naltrexone implant as an outpatient, please make sure you book the appointment for its insertion at least 3 weeks in advance. A £150 deposit payable in order to book the appointment and this forms part of the total cost of the implant. The deposit is only refundable if you cancel the appointment by giving at least 4 days notice. If you cancel or rearrange the appointment with less than 4 days notice, or you do not attend for the appointment, the deposit will be lost and you will need to pay a further £150 before another appointment is made.
